Contact & booking

Tell us what
is going on.

The fastest route is the phone: Bev answers it and can usually put you in a chair the same week. The form below reaches the same desk and gets a call back the same working day.

Booking request

Send this and we will call you.

Nothing here is a confirmed appointment until someone from the desk has spoken to you. Please do not use this form for anything urgent.

For example: Tuesday or Thursday mornings, or any Friday before noon.

Which tooth, how long, what makes it worse, what you have already been told elsewhere. Insurance carrier too, if you have one.

Fields marked are required. We use what you send here to book your visit and nothing else.

Getting here

Two blocks off the Halstead commercial strip, on the corner with Weller. Free surface lot behind the building, entered from the alley; there is no permit and no time limit. Route 14 stops directly at the door, both directions. The entrance is at street level with no step.

If it is urgent

After hours, and when to skip us entirely.

Call (612) 555-0176 and the recording gives you the on-call number. One of the two dentists carries that phone every night and weekend. Call it. It is not an answering service and nobody is annoyed to hear from you at nine on a Sunday.

Weekday mornings we hold two chairs empty. Call before 10 AM and you will almost always be seen that day.

Do not call us first if there is facial swelling that is affecting your breathing or your swallowing, if the swelling is closing an eye, or if you have a fever with it. That is an emergency room, not a dental chair. The same goes for a jaw injury after a fall or a crash, or bleeding you cannot stop after twenty minutes of firm pressure. Go, and call us afterwards; we will pick up the tooth once you are safe.

If you send the form after 3 PM on a Friday it will be Monday morning. Nobody is reading email over the weekend, which is why the on-call number exists.

First visit paperwork

Bring the card, not the folder.

Your insurance card and a list of anything you take daily. If you have X-rays from another practice taken in the last two years, email us the name of the office and we will request them, so you are not carrying a disc around.
